1. YesNo I agree to hold "The Permanent Makeup Studio" harmless if I decline a patch test.

2. YesNo I understand the risks associated with the brow tint and lamination service, I understand that with this procedure I may experience skin or eye irritation, eye pain, eye itching, discomfort, or swelling.

3. YesNo I understand that my skin and eyebrows may react to this service and may experience over pigment or lack of pigment, residual staining of the skin, or damage to the natural eyebrows. I agree that if I experience any of these issues, I will contact "The Permanent Makeup Studio" and consult a physician at my own expense.

4. YesNo I agree to follow the care instructions provided to me by "The Permanent Makeup Studio" for the care of my tinted and/or laminated eyebrows.

5. YesNo I understand that there is no guarantee of how long my eyebrows will remain tinted or laminated.

6. YesNo I understand I must not wet, steam, or put on makeup for at least 24 hours after the service is performed.

7. YesNo I understand that I will be lying in a reclined position and if have a medical condition that might be aggravated by lying still for this period will inform "The Permanent Makeup Studio" of such condition and will not be able to have the brow tint performed to my eyebrows.

8. YesNo This agreement will remain in effect for the procedure and all future reoccurring procedures of the same nature.
